Care Navigation
Care navigation offers individuals choice by providing information to help them move through the health and social care services and access the support that is right for them. Our reception staff have been trained to support patients by signposting them to the most appropriate professional or service.
We can currently signpost to the following external services:
- Extended Access - evening or weekend appointments in our hub clinic
- Community Pharmacy
- First Contact Practitioner (FCP)
- Social Prescribing Link Worker (SPLW)
- Mental Health Practitioner

- Stop Smoking
